Code of Ministerial Conduct & Acknowledgement – may be downloaded and sent to the Parish Center or may be completed online. Code of Ministerial Conduct & Acknowledgement - Diocese of Worcester - Worcester, MA (worcesterdiocese.org). We also have copies in the Parish Center. This needs to be done once unless there is an update.

Safe Environment Training - may be completed in the Parish Center or online. Please contact Julie Basque or Melissa Baril-Lower for training.
This needs to be done only once.

CORI Background Screening - may be completed by downloading the form here: CORI - Background Screening - Diocese of Worcester - Worcester, MA (worcesterdiocese.org)

Please fill out the form and send it to the Parish Center along with a copy of your license. We also have copies in the Parish Center. This needs to be done every five years.
